Evergreen Palliative Care Services provide consultative care to bridge the gaps often experienced by chronically ill people living with progressive, complex, life-limiting conditions.  Our dedicated staff consult with the patient, family and the patient's professional health care providers to evaluate treatment options, identify support systems and assist with care coordination between the multiple disciplines serving the patient.
 
We are seeking a Palliative Care Nurse Practitioner to provide consultative services for patients and healthcare providers in the community regarding decision-making and symptom management at the end of life. The NP will visit patients and provide education in the outpatient and inpatient settings under the supervision of the Palliative Care Medical Director. The NP will routinely see patients in the Skilled Nursing Facilities, Assisted Living Facilities and private residences in King and Snohomish counties.
 
Requirements:
 
*Current State of Washington professional nursing license and ARNP license.
*Advanced practice certification.
*Minimum of one year experience in Hospice, Palliative Care, or related field experience as an ARNP.

Job Summary
The Hospice Home Care RN is responsible for the total direct nursing care of all patients within established standards of care, hospital and unit policies, procedures, and guidelines.  The Hospice Home Care Nurse continuously plans, implements, and evaluates patient care, incorporating patient and family into the process, provides direct patient care, and functions as a patient advocate.

Qualifications and Requirements

Education Background
Bachelor of Science in Nursing preferred.

Work Experience
Varies by nursing department
Minimum score of 70% on pretest examination (if applicable to area)

Motor Vehicle Experience/Record
Some RN positions require driving.  All drivers must be 18 years of age and have been driving for a minimum of two (2) years, although 21 years of age is preferred.  All drivers must have a current, valid Washington State Drivers License and current auto insurance.

Position Summary:
The Hospice and Palliative Care Manager is responsible for the overall management, marketing, and financial success of the Hospice and Palliative Care Programs. This management position is responsible for supervision of the clinical and administrative staff. It requires leadership skills, independent thinking, organization, interpretation of information relating to a wide variety of policies and procedures, data analysis, coordination and delegation of work assignments, planning and prioritization. The manager is responsible for annual budget preparation and monitoring to ensure that expenditures are in compliance with approved budgetary constraints.

Minimum Requirements:
Masters degree in Nursing, Business, Public Health or related field. Bachelor’s degree and six (6) years experience may substitute for graduate degree.
- Three (3) years Medicare Certified Hospice or Home Health experience required.
- Three (3) years experience in progressively more responsible leadership/management roles.
- Required to have a valid driver’s license and proof of current auto insurance.
- Current WA nursing licenses.
- Current CPR.
- Proof of TB screening.
- HIV/AIDS education

Strong clinical background is very important.

• Provide psychosocial assessment, counseling, and discharge planning for long term care.

• Work collaboratively with other members of the patient-care team to ensure successful care management

Required qualifications for this position include:
